Synaptic transmission occurs when neurotransmitters reach the axon terminals of the presynaptic cell. They are then released into the synapse and travel across to the postsynaptic cell. On the postsynaptic cell they bind to neural receptors on the surface of the cell. This initiates communication between the two neurons and a neural impulse is sent down the postsynaptic cell. Any neurotransmitters left in the synapse are reabsorbed into the presynaptic cell through the reuptake mechanism.
